![]() ![]() Topical nitroglycerin (0.2% cream applied twice daily) has been used with success however, we have found that recurrence is common and patients often complain of headaches that accompany its use. We find that topical diltiazem (2% gel) may relieve the pain and spasm and promotes healing. Because ischemia has been implicated in the propagation of a chronic fissure, several topical smooth muscle relaxants have been shown to be effective, probably by both lowering resting anal pressure and promoting vasodilatation. Topical corticosteroids and suppositories are ineffective and should be avoided topical anesthetics are popular, but we do not routinely recommend their use to our patients. Warm sitz baths seem to provide comfort and may relax the anal spasm and provide gentle anal cleansing following bowel movements. Standard treatment includes stool softeners, such as methylcellulose or dietary fiber. For this reason, reassurance is beneficial-emphasize that most fissures heal spontaneously with nonoperative treatment. Your patient may well express some anxiety about the pain and bleeding associated with an acute anal fissure. Fissures that are not close to the midline suggest other processes that could result in anal or perianal ulceration, such as Crohn disease, syphilis, herpes, and carcinoma. Up to 10% of fissures occur in the anterior midline (particularly in women). At this point, further examination of the anorectum with either a finger or instrument is unnecessary and can be quite painful for your patient. You can easily diagnose an anal fissure by carefully spreading the buttocks apart and observing the linear ulceration in the anal canal. Thus, the classic triad of a chronic anal fissure is the fissure itself, accompanied above by a hypertrophied anal papilla and below by a sentinel skin tag. The anal papilla at the cephalad edge of the anal fissure may also become enlarged and hypertrophied as a result of the chronic inflammation. This swollen epithelial tag may resemble an external hemorrhoid. At this point, the adjacent perianal skin may be edematous, forming a "sentinel pile"-the hallmark of a chronic fissure that will likely need surgical treatment. 1 Often by the time patients arrange an appointment, what started as a tear has become an ischemic ulceration. This syndrome is associated with relative ischemia of the posterior midline of the anal canal, an area with a relative paucity of blood vessels. The pain causes sphincter spasm and anal hypertonia, and thus begins a vicious circle of pain, spasm, and constipation. ![]() The linear injury is typically caused initially by the passage of a hard stool, resulting in pain with defecation that can be accompanied by bleeding. In men and most women, the tear, or ulcer, occurs very close to the posterior midline of the anal canal. FISSURE An anal fissure is simply a tear in the anoderm. ![]() In this article, we discuss the diagnosis and treatment of these conditions, as well as other, less common causes of anal pain. In fact, 95% of patients who complain of anal pain have one of the following 3 conditions: anal fissure, thrombosed hemorrhoid, or anorectal abscess. While it is certainly understandable that our patients should make such an assumption, it is all the more important for us, as health care providers, to remember that anal pain frequently has a nonhemorrhoidal cause. They frequently phone and ask us to prescribe a medication for their "painful hemorrhoids," and they are often dismayed when we insist that they must come to the office for an examination before we can prescribe any treatment. Patients almost always believe that their anorectal problems are caused by hemorrhoids, regardless of the nature of their symptoms. Homeowners with small yards and few weeds would do well to save the money find a good corded or battery-powered weed eater. Small yards don’t require the same maintenance as large plots of land. The kind of weed eater you need will, to a large extent, depend on the land area you have, and what you want to do with it. ![]() ![]() Rated at 350 horsepower, this mighty mouse sported factory hardware that rivaled even the best stuff down at the local speed parts emporium. The new Nova Super Sport was available with the economical six cylinder, or myriad mouse-motor derivatives, but clearly the hot ticket was the L79 version of the 327. It, too, underwent a complete restyling in 1966 into what many Nova aficionados consider the most desirable example of the marquee. ![]() Hot on the heels of the newly restyled Chevelle came the Chevy II Nova. As a true performance platform, the Nova was finally beginning to come around. A slight restyling put the turn signals in the front bumper rather than in the grille, while out back a new taillight treatment could be found. Bucket seats, a floor shift and a console were expected ingredients included in the SS package, as were the Nova SS badges on the quarters and rear cove area. Still available on the last of the first-generation Novas, the Super Sport package remained virtually unchanged from the preceding years. It was also the first year for the twelve-bolt posi rear, an in-dash tachometer, and a FM band radio. Not only was it the first year for the 327 cid engine but is was the only year a Powerglide transmission could be ordered with a high performance 327. The 1965 Nova was significant for many reasons. Both the 250 and 300 horsepower variants (元0 and L74 respectively) could be specified on the order form, and when coupled to the optional close-ratio four-speed and a set of 3.36 gears, the Nova was an unpleasant surprise for many unsuspecting stoplight bandits. The 327 was deemed as an acceptable means of motivation for the Nova and was added to the option list. The Nova was the willing recipient of an even larger infusion of horsepower in 1965. ![]() With the introduction of the factory-installed, 195 and 220 horsepower 283 small blocks (元2 and L77) in 1964, the 2,500 pound Nova began taking on the image of a real musclecar. This made the 1963 convertible SS one of the most valuable Novas, even though it only came with the 194-ci 6 cylinder. This was the only year that Chevrolet built a "drop-top" Nova SS because it discontinued the convertible body style on Novas in 1964. But while the rest of the Chevy lineup was intoxicated by the heady horsepower generated from big blocks, the Nova maintained its compact car status and plucked its performance from the 153-ci in-line 4-cyl and the 194-ci straight-6.Ī Super Sport option (RPO-Z03) first became available on the Chevy II Nova 400 line in 1963. ![]() That engine would later be used as a base for the development of GM's 4-cylinder sub-compact engines of the late 70's and 80's.Ī strong following had been generated by Chevy's diminutive compact since its introduction in 1962, thanks, in no small part, to its sensible size and sporty appeal. It was the first car to offer a Chevrolet 4-cylinder engine since 1928. The 153-ci 4-cyl and the 194-ci 6-cyl were all new engines when the Nova debuted on September 29, 1961. Production began in August of 1961 and as the Chevy II gradually became available with many engine, trim, and convenience options, it joined in the performance ranks of the mid-60's as an inexpensive musclecar. The Chevy II was designed as a relatively plain intermediate "economy" car powered by four and six cylinder engines, to steal sales away from the popular Ford Falcon. The plans for the Chevy II began in December of 1959 with the code name H-35. It offered buyers a third choice in terms of size, standing between the (then) compact Corvair and the full size Chevrolet. In the Northern Hemisphere, the March equinox is called the vernal or spring equinox while the September equinox is called the autumnal or fall equinox. As a consequence, according to a properly constructed and aligned sundial, the daytime duration is 12 hours. Long before conceiving this equality, primitive equatorial cultures noted the day when the Sun rises due east and sets due west, and indeed this happens on the day closest to the astronomically defined event. They are not exactly equal, however, because of the angular size of the Sun, atmospheric refraction, and the rapidly changing duration of the length of day that occurs at most latitudes around the equinoxes. ![]() On the day of an equinox, daytime and nighttime are of approximately equal duration all over the planet. The word is derived from the Latin aequinoctium, from aequus (equal) and nox ( genitive noctis) (night). The instants of the equinoxes are currently defined to be when the apparent geocentric longitude of the Sun is 0° and 180°. In modern times, since the Moon (and to a lesser extent the planets) causes Earth's orbit to vary slightly from a perfect ellipse, the equinox is officially defined by the Sun's more regular ecliptic longitude rather than by its declination. Equivalently, this is the moment when Earth's rotation axis is directly perpendicular to the Sun-Earth line, tilting neither toward nor away from the Sun. More precisely, an equinox is traditionally defined as the time when the plane of Earth's equator passes through the geometric centre of the Sun's disc. This occurs twice each year, around 20 March and 23 September. On the day of the equinox, the Sun appears to rise "due east" and set "due west".
